6. Health Center/Doctor's Certificate for health and sanitation examination

7. Application letter for food/beverage production permit to the Health Service

8. Data on food/beverage products produced (accompanied by samples of production results

food/drink)

9. Labels that will be used on food/beverage products produced

10. Laboratory test results recommended by the Health Service
