- The name is not the same/similar to the name of the state institution,

government/international institutions, unless permission is obtained

- Does not consist of numbers/chains of numbers, letters/chains of letters

does not form words

- Minimum three syllables (in Indonesian)

2. Business Address/Domicile

- Proof of Business Place Ownership (Certificate, PBB, IMB,

AJB/PPJB), and/

- Lease Agreement Letter

- Photos of the exterior and interior

3. Structure:

a. The management consists of:

- Chief

- Secretary

- Treasurer

b. Supervisors consist of:

- Chief

- Member

c. Coach:

- Chief

- Member

d. Founding father

4. Initial capital min. Rp. 10,000,000

5. Aims & Objectives and Activities of the Foundation (Humanitarian, Social &

Religious) according to the 2020 KBLI

6. Photocopy of the identity of the Management, Supervisors, Trustees and Members (E-KTP

& NPWP that is validated and active and clear of Tax & SPT arrears
