Establishment Requirements:

1. Name of the Association

- The name is not the same/similar to the name of the state institution,

government/international institutions, unless permission is obtained

- Does not consist of numbers/chains of numbers, letters/chains of letters

does not form words

- Minimum three syllables (in Indonesian)

2. Business Address/Domicile

- Proof of Business Place Ownership (Certificate, PBB, IMB,

AJB/PPJB), and/

- Lease Agreement Letter

3. Structure:

a. The management consists of:

- Chief

- Secretary

- Treasurer

b. Supervisors consist of:

- Chief

- Member

4. Initial capital min. Rp. 10,000,000

5. Aims & Objectives and Association Activities

6. Draft Articles of Association/Bylaws of the Association (which

already made internally if any)

7. Photocopy of the identity of the Management, Supervisors and Members (E-KTP & NPWP

which is validated as active and clear of Tax & Annual SPT arrears

Personality of each Management and Supervisor min. last 2 years)

8. Letterhead & Stamp (when available)
